The Value of Computerized Tomographic Scanning in Ruptured Intracranial Aneurysm



Abstract
The cranial computed tomographic scans of 35 cases of proved ruptured aneurysm were analyzed for the prognostic significance in relation to the clinical grade and severity of vasospasm.
CT scans demonstrated blood clot in subarachnoid space, parenchym and ventricles in 18 cases (51.4%), 8 cases (22.9%) and 4 cases (11.4%) respectively. The suprasellar cisterns, interhemispheric fissures and sylvan cisterns were most common location where subarachnoid hemorrhage was frequently detected. In cases complicated with intracerebral hemorrhage, hemorrhage into frontal lobe and septum pellucid was peculiar for anterior communicating aneurysms, and scans showing hemorrhage into temporal lobe and sylvan fissure alone were obtained with middle cerebral aneurysms. The incidence of ventricular dilatation was 31.4%. Aneurysms were visualized on scans in most cases with larger aneurysms than l0mm in diameter. There was a tendency that the vasospasm developed frequently and severely in the cases having diffuse subarachnoid hemorrhage on scans of acute stage.
